CONSENT STATEMENT FOR THE PROCESSING OF PERSONAL DATA

In accordance with the ‘Personal Data Protection Law’ No. 6698 and the ‘Regulation on the Processing of Personal Health Data and the Protection of Privacy,’ I have been provided with training regarding the relevant legal regulations and the rights I possess within the company. I have read and reviewed the information document delivered to me following the training. Within the scope of the relevant laws, I hereby give my consent to SMARTES TEKNOLOJİ YAZILIM DANIŞMANLIK SAN. TIC. LTD. ŞTİ. to process my personal data, including those that identify me or can be used to identify me, which I have provided verbally, in writing, and/or electronically, including special category personal data and health data, being processed by SMARTES TEKNOLOJİ YAZILIM DANIŞMANLIK SAN. TIC. LTD. ŞTİ. for the purpose of providing the service and planning such services, and being shared with other persons and organisations to whom such sharing is necessary in accordance with the relevant legislation; I consent to the processing of my personal data, special category personal data, and health data within the scope defined in the ‘Law on the Protection of Personal Data’ No. 6698 and the ‘Regulation on the Processing of Personal Health Data and the Protection of Privacy,’ as detailed below.

• Data relating to a natural person whose identity is known or can be determined, processed either partially or fully automatically or non-automatically as part of a data recording system, containing information about the person’s identity;

Documents such as driver’s license, identity card, and passport containing information such as first name, last name, Turkish ID number, nationality, mother’s name, father’s name, place of birth, date of birth, gender; Criminal Record, Diploma Sample, Disability Certificate (if required); Military service status certificate, health report, blood type card, biometric data, marriage certificate and tax number if married, Social Security Institution (SGK) number, credit card information, bank account information, signature information, vehicle registration number, names and surnames of spouse and children, Turkish ID numbers, name, surname, and phone number of a close relative to be contacted in case of emergency,

• Data belonging to an identified or identifiable natural person; processed partially or fully automatically or non-automatically as part of a data recording system; communication information; phone number, address, email address, fax number, IP address, etc.,

• Data that is clearly associated with a specific or identifiable natural person; processed either partially or fully automatically or non-automatically as part of a data recording system; marketing data containing purchase history information, survey results, cookie records, information obtained through campaign activities, etc.

• Data that is clearly attributable to an identified or identifiable natural person; processed either partially or fully automatically or non-automatically as part of a data recording system; data obtained through record-keeping for security and workflow purposes;

Personal data obtained from card-based or cardless access systems and documents upon entry into a physical location or during stay within a physical location; camera recordings and recordings taken at security points, vehicle tracking system recordings in the event of company vehicle use, access logs related to the use of company computers,

• Personal data that is clearly attributable to an identified or identifiable natural person; processed in part or in full by automated means or in a non-automated manner as part of a data recording system; personal data related to the receipt and evaluation of any requests or complaints directed to SMARTES TEKNOLOJİ YAZILIM DANIŞMANLIK SAN. TIC. LTD. ŞTİ.

I have read the Information Notice regarding the Protection and Processing of Personal Data, which is also available on the website of SMARTES TEKNOLOJİ YAZILIM DANIŞMANLIK SAN. TIC. LTD. ŞTİ., in both printed and electronic formats, and I consent to the processing of my personal data in the manner described above, including its recording, storage, stored, retained, reorganised, shared with institutions authorised by law to request such personal data, and transferred, disclosed, transferred, classified, and processed in any other manner as provided by the Personal Data Protection Law (KVKK) to third parties within or outside the country, in accordance with the conditions set forth in the KVKK, I hereby freely consent.
